Injection of botulinum toxin into small muscles of the face, scalp, and neck causes those specific muscles to hold their function or be paralyzed, thereby improving the appearance of fine lines and wrinkles. I understand the goal is to decrease the wrinkles in the treated area. I understand that I will be injected with a botulinum toxin, Botox/JEUVEAU, in the area of the glabella, forehead, crows feet, or other facial, scalp, or neck area, to paralyze the facial, scalp, or neck muscles temporarily. This paralysis is temporary and re-injection is necessary within 3 to 4 months. No guarantee, warranty, or assurance has been made to me as to the results that may be obtained.
